This was made in V3 but saved as V2 so there shouldn't be any problems opening it. I haven't added the perforated line because I don't like the look of it but it's easy to do if you prefer it.

A couple of people have said they can't open the file (I'm not sure why) so I've changed it and hopefully it's ok....please let me know
