I use Sakura with the ordinary pen holder.
If you want to make your own, insert the pen into the pen holder to see where on the barrel it stops then take a craft knife and just shave a little bit off the barrel at that point, it isn't much so don't go mad, then keep inserting it and shaving a little until it fits. Then all you do is swap the inserts depending what colour you want to use.
It is a one of job and cheaper than £15.
Here you go it isn't easy to see on the barrel, just in case this helps as well the pen top still fits snug.
